We at Valley Women’s Health Group are reaching out to inform you about significant upcoming changes to our practice. After 25 years of dedicated service, we are embarking on a new chapter that we believe will enhance our ability to provide you with exceptional women’s healthcare.
We are thrilled to announce that Doctors Lilibeth Denham, Peilan Tang, and Nicole Thompson will continue their medical practices by joining Baystate Women’s Health, located at 3300 Main Street, Springfield, MA, effective June 17, 2024. This transition marks an exciting opportunity to integrate our expertise with a leading healthcare system, ensuring continued high-quality care for our patients.
However, alongside this exciting news, we also announce the retirement of Dr. Devon Foulks, effective June 30, 2024. Drs. Yelena Mikich and Hani Haddad have previously announced their retirements. For patients of these retiring doctors who wish to continue receiving care at our new Pioneer women’s health practice within Baystate Women’s Health, or for those seeking to transfer their care to another provider, we have enclosed a Medical Record release form and detailed instructions later in this communication.
We warmly invite you to join us at Baystate Women’s Health. To schedule an appointment at our new office, please call our dedicated NEW office line at (413) 794-7045, starting May 20, 2024. Our team is ready to assist you with your healthcare needs in this new setting.
For patients of Dr. Denham, Dr. Tang, or Dr. Thompson with appointments scheduled after June 14th at Valley Women’s Health Group, please note that these appointments will be cancelled. To ensure a seamless continuation of your care with us at Baystate Women’s Health, kindly complete the enclosed record release form. We will then coordinate the secure transfer of your medical records to our new location.
For our valued obstetrics patients, we are committed to ensuring a smooth transition of your care. We will work closely with you to make this process as seamless as possible, prioritizing your comfort and well-being throughout your pregnancy journey.
Facilitating Your Transition to a New Physician/Provider:
- Medication Refills: To assist with your transition, we will continue to process medication refills until May 31, 2024. Please continue to submit your refill requests through your pharmacy as usual.
Accessing or Transferring Your Medical Records:
- Prior to June 14, 2024: To obtain a copy of your medical records or arrange for a transfer to another provider before June 14, 2024, please complete the enclosed medical release form and mail it to our Valley Women’s Health Group office at 3550 Main St, Suite 302, Springfield, MA 01107, or fax it to 413-737-8540.
- After June 14, 2024: For medical records access or transfer requests after June 14, 2024, please send a signed written request to Baystate Health, Attn: Health & Information Management, 759 Chestnut Street, Springfield, MA 01199.
